The Exchange

If you are looking for records from an outside organization, you will want to use the The Exchange section of the secure web portal

1. Log into the Centralis Health secure web portal and click on The Exchange in the left menu bar

**If you do not have the The Exchange option contact your account designee to review your access to this functionality

2. Enter all required fields (Last Name, First Name, Gender) and DOB to narrow the search on common names,  then click Find Patient

3. Review the results, and after confirming you have the right patient select it by clicking their  hyperlinked name

4. If you have never viewed this patient before in the Centralis Health web portal then you will see the below attestation, you must click Accept & Proceed to move on to the next page

5. You are now on the Patient Home screen, the records highlighted in Centralis blue squares are available records from Centralis Health participants who are connected in your healthcare community. 

6.  Select the record time you wish to view by clicking the Centralis blue box, then locate the record you wish to view and click the hyperlinked title to display record

7. Once you've located a record you need you can scroll to the bottom of the report and will see the options to Print or Download record
