Submitting Document Attachments

You will first log into Centralis Health secure web portal and navigate to your Received Items Requiring Action



Open the item by clicking on the blue hyperlink under the patient or the # column.



First you will link a record to an existing ModMed - EMA chart by typing the patient Last, First name and selecting the correct patient from the drop down


After clicking Save Details confirm the patient is linked to the correct MR #


If you are ready to submit the record to the patient's chart, you will navigate to the ModMed tab at the top of the screen.



Select a category and then click submit.




If you have no other workflow tasks to complete in Centralis you can then mark the item as Done to be taken back to your active work queue list. 


Records submitted using a category labeled as "Results - " will be located in the Results section of ModMed for review. 

***Please note you must add the performed date IN MODMED as the API does not currently support that option, this should be available soon. 



All other documents can be located in the patient chart under the Attachments section.
