How to Check Patient Eligibility

1. Log into the Centralis Health web portal and select The Exchange from the left hand menu. 

2. Enter the patients Last Name, First Name, DOB, and Gender. Select Find Patient.

3. Confirm you have found the correct patient by hovering over their name, this will show a list of all "sources" the patient has data for. If records are available from Manatee County (like seen above), select desired patient record.

4. From the patient home screen, select Demographics.

5. Click on the patient name next to the Manatee County Indigent Care source. 

6. Confirm Eligibility.
